29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) New residential construction located on Lot # 156 Silver Birch Pass, Newnan, GA 30265, Second floor top plates: On or about 12/16/2021, the employer exposed employees to overhead struck-by hazards, in that head protection was not provided to employees engaged in roof truss installation activities using a crane to set each truss.   b) New residential construction located on Lot # 156 Silver Birch Pass, Newnan, GA 30265, Front street and west side of structure: On or about 12/16/2021, the employer exposed employees to overhead struck-by hazards, in that head protection was not provided to employees engaged in roof truss installation activities using a crane to set each truss.   c) New residential construction located on Lot # 156 Silver Birch Pass, Newnan, GA 30265, Crane operator's cab: On or about 12/16/2021, the employer exposed employees to overhead struck-by hazards, in that head protection was not provided to the crane operator who was hoisting roof trusses directly overhead without a protective helmet.

29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   New residential construction located on Lot # 156 Silver Birch Pass, Newnan, GA 30265, Second floor top plates: On or about 12/16/2021, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns, in that protective eyewear was not provided to employees engaged in roof truss installation. Employees were exposed to flying collated plastic debris, wood debris, and metal fasteners with each pull of the trigger.

29 CFR  1926.416(e)(1): Worn or frayed electric cords or cables were used:   New residential construction located on Lot # 156 Silver Birch Pass, Newnan, GA 30265, Front side of structure and street: On or about 12/16/2021, the employer exposed employees to electrical shock/burn hazards while using electrical cords to power equipment on site, in that the damaged cords were not removed from service immediately as the cords used were beyond repair and showing bare internal circuits as the covers were torn and frayed.

Safe access was not provided to employees accessing the second floor landing as employees climbed 2x4 cleats.
