29 CFR  1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:   a)  At 1131 W Romana Street, Lot 4 Romana Place, Pensacola, Florida:  On December 17, 2021, the employer allowed his employees to perform framing activities from a job made scaffold with a walkboard that measured 12 inches wide not 18 inches wide as required by the standard exposing his employees to a fall hazard of 16 feet to the ground below.

29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a)  At 1131 W Romana Street, Lot 4 Romana Place, Pensacola, Florida:  On December 17, 2021, the employer allowed his employees to perform framing activities from a job made scaffold without fall protection exposing his employees to a fall hazard of  16 feet to the ground below.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   a)  At 1131 W Romana Street, Lot 4 Romana Place, Pensacola, Florida:  On December 17, 2021, the employer allowed his employee to perform framing activities from a job made scaffold without first ensuring his employee was trained on the hazards associated with scaffolds to include but not limited to erecting scaffolds and inspecting scaffolding exposing his employees to a fall hazard of 16 feet to the ground below.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)  At 1131 W Romana Street, Lot 4 Romana Place, Pensacola, Florida:  On or about December 17, 2021 and at times prior, the employer allowed his employees to perform framing activities without fall protection exposing his employees to a fall hazard of up to 21 feet 3 inches to the ground below.  The Gaston Gonzalez dba Villedas Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(B)(13) which was contained in OSHA inspection number 1306505, citation number 1, item number 1 and was affirmed as a final order on June 27, 2018, with respect to a workplace located at 12419 Squirrel Drive, Lot 90 Grace Magnolia, Spanish Fort, Alabama.   The Gaston Gonzalez dba Villedas Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(B)(13) which was contained in OSHA inspection number 1360747, citation number 2, item number 1 and was affirmed as a final order on February 5, 2019, with respect to a workplace located at 12508 Squirrel Drive, Lot 86 Grace Magnolia, Spanish Fort, Alabama.
