29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in an in-patient hospitalization, amputation, or the loss of an eye. (a)On or about December 11, 2021, the employer was notified of an employee's hospitalization due to COVID 19 VIA text message. The Hospitalization was not reported to OSHA until Friday, December 17, 2021, at approximately 10:40 AM. When the by the facility Safety Manager called to report the hospitalization.
