29 CFR  1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices:   a)  In the Machinery Room, on or about December 22, 2021 - The employer did not document that equipment complied with Recognized and Generally Accepted Good Engineering Practices (RAGAGEP) such as IIAR 2 (2014) Standards for Safe Design of Closed-Circuit Ammonia Refrigeration Systems Section 6.7 when employees did not have access to an eyewash/safety shower unit located inside the machinery room.  The facility maintenance mechanic performs work in the machinery room.  Failure to provide an accessible eyewash/safety shower in the Machinery Room can expose employees to serious eye damage and severe burns following skin contact.

29 CFR  1910.119(d)(3)(i)(E): Information pertaining to the equipment in the process did not include a ventilation system design:  a)  In the Machinery Room, on or about December 22, 2021 - The employer did not document ventilation system design basis, such as but not limited to applicable codes and standards, methods used to determine the maximum rate that the ventilation system must exhaust (e.g., cubic feet per minute (cfm)) for both emergency and non-emergency ventilation systems, and the supporting calculations for the ventilation system.  The unexpected release of anhydrous ammonia or a loss of air circulation could result in the accumulation of ammonia vapors inside the machinery room exposing employees to fire, explosion, and inhalation hazards.

29 CFR  1910.119(j)(2): The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment:   a) In the facility, on or about December 22, 2021 - The employer did not establish and implement procedures to maintain the on-going integrity of process equipment, when the written procedures did not include procedures for visually inspecting insulated ammonia vessels, such as but not limited to -49-degree Recirculator Vessel; -28-degree Recirculator Vessel; and +15-degree Recirculator Vessel for damage and/or moisture inside insulation and procedures for testing vessel at area where insulation degradation may be observed.  Failure to inspect, test and maintain insulated ammonia vessels can lead to loss of containment and exposure to toxic vapors and fire hazards.  b) In the facility, on or about December 22, 2021 - The employer did not establish and implement procedures to maintain the on-going integrity of process equipment, when the written procedures did not include procedures for visually inspecting insulated ammonia piping, such as but not limited to high temperature recirculated suction; low temperature recirculated suction; high temperature recirculated liquid; high gas defrost; and low temperature recirculated liquid for damage and/or moisture inside insulation and procedures for testing piping at area where insulation degradation may be observed.  Failure to inspect, test and maintain insulated ammonia piping can lead to loss of containment and exposure to toxic vapors and fire hazards.
