1910.120(q)(3)(v) The individual in charge of the ICS did not limit the number of emergency response personnel at the emergency site, in those areas of potential or actual exposure to incident or site hazards, to those who are actively performing emergency operations. However, operations in hazardous areas shall be performed using the buddy system in groups of two or more. On or about December 23, 2021, employees at ExxonMobil Refinery in Baytown, Texas, not all responding employee personnel used the buddy system to be accompanied by ERT personnel when entering the unit during an emergency to isolate/close valves in order to block-in fuel after certain remote operated valves became inoperable as a result of a fire.

This violation was most recently observed on or about December 23, 2021, in Hydro Desulfurization Unit (HDU-1) at ExxonMobil refinery located in Baytown, TX, when the employer did not ensure Process Safety Information (PSI) such as Piping and Instrumentation Diagram (P&ID) # 134-500-0058, Rev 4, was complete and accurate included, but not limited to the following: a. 14-inch flanges were missing on a bypass line from furnace F-701 to reactor R-702B. b. Line number 0-084-14"-300# was missing for 14-inch bypass line. c. Normally closed (NC) symbols were missing for valves ROV-075 and ROV-605.
