29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred:  On or about March 21, 2022:  a. At the facility where employee #1 was diagnosed on January 12th 2022 with a work related mental illness by a licensed health care professional resulting in days away from work and prescription medication that was not listed on the 2022 OSHA 300 Log or an OSHA form 301 within seven calendar days of the employee voluntarily providing the documentation to the employer.  b. At the facility where employee #2 was diagnosed on January 17th 2022 with a work related mental illness by a licensed health care professional resulting in days away from work that was not listed on the 2022 OSHA 300 Log or an OSHA form 301 within seven calendar days of the employee voluntarily providing the documentation to the employer.  c. At the facility where employee #3 was diagnosed on February 1st, 2022 with a work related mental illness by a licensed health care professional resulting in days away from work and prescription medication that was not listed on the 2022 OSHA 300 Log or an OSHA form 301 within seven calendar days of the employee voluntarily providing the documentation to the employer.
