At the facility where employee #3 was diagnosed on February 1st, 2022 with a work related mental illness by a licensed health care professional resulting in days away from work and prescription medication that was not listed on the 2022 OSHA 300 Log or an OSHA form 301 within seven calendar days of the employee voluntarily providing the documentation to the employer.
