29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:  location:  building perimeter, 3rd level on or about: 12/28/21  a) Employees were tied off to wire ropes that were installed as part of a guardrail system. The employees were exposed to a fall of 37 feet.  NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.
