29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a. 135 national Rd., Edison, NJ - Palletizer  The employer did not ensure that Lockout/Tagout procedures were utilized by employees performing maintenance/servicing of equipment where inadvertent energization of the equipment could cause serious harm or injury to an employee. Two employees engaged in the troubleshooting and repair activities of a palletizer machine (line 3), entered the danger zone and did not perform the LOTO procedures to secure the machine prior to entry.  Violation observed on or about December 29, 2021  b. 135 national Rd., Edison, NJ - Body Maker - Line 1  The employer did not ensure that Lockout/Tagout procedures were utilized by employees performing maintenance/servicing of equipment where inadvertent energization of the equipment could cause serious harm or injury to an employee. Employees performing maintenance of a Body Maker/Trimmer (#15) opened the machine guards and did not complete LOTO procedures to deactivate the equipment prior to placing his arms into the machine. Employees operated an electric overhead crane, on a regular basis, that was not inspected at intervals that would ensure deficiencies with the crane were identified. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. 135 National Rd., Edison, NJ - Body Maker / Trimmer  The employer did not ensure that methods of machine guarding were provided to protect machine operators from hazards created by point of operation, rotating parts, and/or ingoing nip points. Employees engaged in the manufacture of food grade metal cans operated equipment that included, but was not limited to Belvac and/or Ragsdale/OLK body maker / trimmer machines , #27 where interlocks which render the equipment inoperable if opened were broken or not operating and would allow an employee to place a body part into the moving machine.  b. 135 National Rd., Edison, NJ - Body Maker / Trimmer  The employer did not ensure that methods of machine guarding were provided to protect machine operators from hazards created by point of operation, rotating parts, and/or ingoing nip points. Employees engaged in the manufacture of food grade metal cans operated equipment that included, but was not limited to Belvac and/or Ragsdale/OLK body maker / trimmer machines , #26 where interlocks which render the equipment inoperable if opened were broken or not operating and would allow an employee to place a body part into the moving machine.  c. 135 National Rd., Edison, NJ - Body Maker / Trimmer  The employer did not ensure that methods of machine guarding were provided to protect machine operators from hazards created by point of operation, rotating parts, and/or ingoing nip points. Employees engaged in the manufacture of food grade metal cans operated equipment that included, but was not limited to Belvac and/or Ragsdale/OLK body maker / trimmer machines , #12 where interlocks which render the equipment inoperable if opened were broken or not operating and would allow an employee to place a body part into the moving machine.  f. 135 National Rd., Edison, NJ - Baler  The employer did not ensure that methods of machine guarding were provided to protect machine operators from hazards created by point of operation, rotating parts, and/or ingoing nip points. Employees engaged in the recycling of food grade metal cans from the manufacturing process, operated equipment that included, but was not limited to a Balemaster, Model 4575G-10, S/N 11416B baler, where guards were removed and laying on the floor next the machine, were not secured to the machine, or were missing, allowing employee direct access to moving machine parts.
