29 CFR  1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about December 30, 2021, employees installed trusses, without the use of a fall protection system. Employees were not provided with a training program that included the use and limitation of fall protection, and how to eliminate or minimize fall hazards. This exposed employees to fall hazards of up to 17.5 feet.

29 CFR  1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:  (a) On or about December 30, 2021, two employees worked from the basket of a Genie aerial lift and were not using a body belt restraint or a personal fall arrest system.
