On or about April 30, 2026, located at 2385 Olivesburg Road in Mansfield, Ohio, the employer did not ensure employees were protected from falls with the use of fall protection when working from a barn roof exposing the employees to a maximum fall hazard of approximately 26 feet. b. On or about April 30, 2026, located at 2385 Olivesburg Road in Mansfield, Ohio, the employer did not ensure employees were protected from falls with the use of fall protection when climbing the exterior of a wooden fame wall to access the roof of a barn exposing the employees to a maximum fall hazard of approximately 26 feet.
