On and about 12/22/2021, employees were exposed to welding hazards where the employees who performed welding activities were not provided required training for the personal protective equipment used  in welding activities.

29 CFR  1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present:  391 St. Jude Industrial Park, Marston,  Cast House:  Employer is exposing employees to hazardous conditions where job hazard assessment is not conducted prior to assigning the employees to perform a task. On and about 12/22/2021, two employees were hospitalized when they sustained injuries from an unstable arrangement of a crucible partially lifted on a platform for welding purposes.   A hazard assessment of the placement and the hoisting activities related to the crucible being welded was not conducted prior to starting the welding activities to determine if the placement was stable and safe.  Two employees were injured when the crucible shifted from its original position.

On and about 12/22/2021, employees were exposed to struck-by hazards where there was no certified document to show that hazard assessment was conducted.  Welding of a crucible was conducted in a new location for the first time.

29 CFR  1910.157(g)(1):  An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage firefighting:  391 St. Jude Industrial Park, Marston: Employer is exposing employees to hazardous conditions where the employees are required to use portable fire extinguishers for incipient stage fires but are not provided required training for using portable fire extinguishers. On and about 12/22/2021,  employees were required to use portable fire extinguishers required for welding activities.  The employees were not provided required educational program for the use of portable fire extinguishers for incipient stage firefighting.

On and about 12/22/2021, employees were exposed to hazardous conditions where they performed welding activities using hazardous substances, such as, but not limited to, welding rods.  The hazardous substances were not included in a Hazard Communication Program as required.
