29 CFR  1910.134(c)(1):  A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use.  Jobsite: 391 St Jude Industrial Park, Marston MO 63866  On or about 7/14/2022, NEA Machine and Fabrication, LLC did not provide a written respiratory protection program. Employees are required to use half-face respirators while conducting maintenance in the carbon-bake area.  Employees were observed donning half-face respirators while cleaning aluminum ore near the north storage tank.  Employees did not receive medical evaluations, fit testing, respirator maintenance procedures, or donning procedures for respirator use.

Jobsite: 391 St Jude Industrial Park, Marston MO 63866  On or about 7/14/2022, NEA Machine and Fabrication had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training.
