29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) Lane Excavating, LLC located at 5751 Cleome Road, Lot 74, Westerville, OH 43081: On and preceding January 4, 2022, the employer did not ensure that a stairway, ladder, ramp or other safe means of egress was accessible as to require no more than 25 feet of lateral travel for two employees working in a trench which varied from 4 to 7 feet in depth. Two employees were working in a trench which varied from 4 to 7 feet deep on a residential sewer tie in without a ladder or other means of egress within 25 feet.  The only means of egress was a stairway which was located on the north end of the excavation more than 30 feet from the area that the two employees were working. The following condition exposed employees to struck-by/caught-in hazards.
