29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips, and sparks. On or about January 4, 2022, employees in the fabrication bay on the southside were utilizing a Kasser Legionnaire cutting and slitting machine without the guards in the proper position to protect employees from the point of operation injuries and contact with the blade.

29 CFR 1910.243(c)(3): Abrasive wheels used on the vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel. On or about January 4, 2022, on the south side fabrication shop area, an employee was using a Milwaukee 4 1/2 inch angle grinder without a guard while performing fabrication tasks.
