29 CFR 1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: At the Maintenance Shop, employees engaged in grinding activities at the Dayton 8" Bench grinder were exposed to struck-by and caught in hazards in that the tongue guard was adjusted to approximately 3/4 of inch from the grinding wheel.

29 CFR 1910.243(b)(2): Airhose and hose connections used for conducting compressed air to utilization equipment was not designed for the pressure and service to which they were subjected: At the Palletizer in the Warehouse, employees engaged in production activities were exposed to struck-by hazards in that the correct adaptor was not used to connect the tool, resulting in potential ejection of the tool.

29 CFR 1910.253(b)(4)(i):Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire; At the Maintenance Area, employees engaged in welding activities were exposed to fire hazards from two improperly stored cylinders of oxygen. The oxygen cylinders was not in use and were stored next to an cylinder of acetylene gas.
