29 CFR 1926.1051(a):A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided: a) 402 Main St., Greenfield, MA: On or about January 4, 2022, an employee was observed climbing up the 4 foot tall concrete forms to gain access to the top of and inside of the forms to fill them with concrete. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19 (d) (1).
