29 CFR 1904.29(b)(3):   Each recordable injury or illness was not entered on the OSHA 300 Log  within seven (7) days of receiving information that a recordable injury or illness has occurred:  a) International Game Technology - 44 Peoples Road, Saugerties, NY:     On or about 12/24/2021, a recordable injury,  which involved an employee receiving a broken ankle as a result of being assaulted by a customer at AHZ Minimart Corp. located at  1 Union Road, Spring Valley, NY,  was not recorded on the OSHA-300 Log of Work-Related Injuries and Illnesses for calendar year 2021 within seven (7) calendar days of receiving information an employee was exposed   to a workplace violence incident which resulted in a serious ankle injury on 12/17/21.      The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19, failure to comply will result in additional penalty of $ 1,000.00.
