The employer failed to develop and implement lock out tag out machine specific procedures for the Cell # 13 rotary mold to include steps to de-energize all sources of energy that have the potential of injuring employees during servicing and maintenance activities.

This was most recently observed on or about January 07, 2022. At a worksite located at 1301 Stahl Drive, Warrensburg, Missouri 64093. Employees were exposed to a crushed-by and struck-by hazard. The rotary mold in cell 13 was not provided with machine guarding to prevent employees in the work area from being struck or crushed between the rotary mold and the robotic saw cabinet.
