29 CFR 1910.23(d)(2):29 CFR 1910.23(d)(2): The employer did not ensure that the minimum perpendicular distance from the centerline of the steps or rungs, or grab bars, or both, to the nearest permanent object in back of the ladder is 7 inches (18 cm), except for elevator pit ladders, which have a minimum perpendicular distance of 4.5 inches (11 cm). 33 Lyman St. Westborough, MA 01581: On January 13, 2022 and at times prior, the employer allowed employees to use a fixed ladder on the south side of the facility with distance between the rungs and the wall at 6.75 inches near bottom and 4.5 inches near the hatch. On January 13, 2022, an employee fell and was injured while ascending this fixed ladder.

29 CFR 1910.23(d)(13)(i):29 CFR 1910.23(d)(13)(i): The employer did not ensure that fixed ladders that do not have cages or wells have a clear width of at least 15 inches (38 cm) on each side of the ladder centerline to the nearest permanent object; and 33 Lyman St. Westborough, MA 01581: On January 13, 2022 and at times prior, the employer allowed employees to use a fixed ladder on the south side of the facility with a distance of 12 inches between the ladder centerline and a communications equipment box affixed to the wall. On January 13, 2022, an employee fell and was injured while ascending this fixed ladder.
