Except as provided elsewhere in this section or by other subparts of this part, the employer did not ensure each employee on a walking/working surface 4 feet (1.2 m) or more above a lower level is protected from falling by one of the systems listed in 1910.28(b)(15)(i)-(iii). (a) On or about January 15, 2022, or at times prior, the employer did not ensure that employees working within/from the Steel King SK3600 push back racking system for the purpose of manipulating, adjusting or removing a jammed pallet, were protected from falling to the lower level. This exposed employees to a fall hazard of approximately 20 feet.

29 CFR 1910.178(l)(3)(ii)(C):Load manipulation, stacking, and unstacking; (a) On or about January 15, 2022, and times prior, employees were involved in the unjamming of a pallets from a Steel King SK3600 Push Back Racking System with a Yale Electric Stand-Up ESC040ADN36TQ090 powered industrial vehicle. The employees had not been trained on the correct procedures for manipulating and unjamming of pallets in the Steel King SK3600 Push Back Racking System. This exposed employees to fall hazards and struck-by hazards.
