(a) 3209 Old Hwy 250, Glennville, GA - On or about On January 12, 2022, the employer did not ensure safe operation of an industrial lift truck due to a pallet being used as an unauthorized platform that was not designed  or approved for use with a forklift, there-by exposing employees to fall and struck-by hazards.

29 CFR  1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied.   (a)  3209 Old Hwy 250, Glennville, GA - On or about On January 12, 2022, the employer did not ensure safe operation of an industrial lift truck due to an employee being elevated  and transported horizontally while working on a wooden pallet that was not attached to the forklift, there-by exposing this employee to fall and struck-by hazards.
