Site - On or about March 3, 2023, an employee was exposed to falling approximately 50 feet while working from Genie aerial lift without having a lanyard attached to the basket. b. Site - On or about March 3, 2023, an employee was exposed to falling approximately 30 feet while working from a JLG aerial lift without wearing fall protection. c. Site - On or about March 3, 2023, 3 employees were exposed to falling approximately 10 feet while working from a JLG aerial lift without having a lanyard attached to the basket while going from the roof of the building to the ground.

29 CFR 1926.760(d)(1): Guardrail systems, safety net systems, personal fall arrest systems, positioning device systems and their components did not conform to the criteria in 29 CFR 1926.502 (Appendix G to this subpart). a. Roof - On or about March 3, 2023, employees were exposed to falling up to 50 feet while decking. The fall arrest harnesses worn by employees were not attached to anchorage points with lanyards. G&M Steel Erection LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.760(d)(1), which was contained in OSHA inspection number 1603377, citation number 01, item number 001 and was affirmed as a final order on 12/10/2019, with respect to a workplace located at 6511 43rd St, Pinellas Park, FL. 33781, Building #18, Unit #1802.
