29 CFR 1926.150(a)(1): The employer did not develop and maintain a fire protection program to be followed throughout all phases of construction and demolition work, and/or did not provide for the firefighting equipment as specified in this subpart:  a-281 E Crescent Ave (Building Exterior) : Employees engaged in roofing activities at this site were exposed to fire hazards from gasoline fueled Twin Tank Rolair compressor onsite. The employer did not ensure that employees had available firefighting equipment at the jobsite.  "Condition observed" or about 01/12/2022.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet  above the upper landing surface to which the ladder was used to gain access:    a-281 E Crescent Ave, Mahwah NJ 07430:  An employee engaged in roofing activities using a portable aluminum extension ladder was exposed to fall hazards of approximately 15ft as the ladder side rails did not extend 3ft above the upper landing surface. Condition observed on or about 01/12/2022

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:    a-281 E Crescent Ave, Mahwah NJ 07430: (8 on 12 pitched roof) An employee engaged in roofing activities activities on a 8 on 12 pitched roof was exposed to fall hazards of approximately 20ft to the ground below in which a rope was used as means of fall protection had not been utilized (Photo #7968). Condition observed on or about 01/12/2022
