29 CFR 1926.502(d)(16)(iii):Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a. Weatherbee Roofing, LLC at the worksite located at 5916 Gulliver Ln in Toledo, Ohio: On or about January 19, 2022, employees performing roofing operations on a residential structure were not adequately protected from falls of approximately 9.5 feet in that personal fall arrest systems did not prevent free falls more than six feet and/or prevent contact to lower levels. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
