29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. At the workplace located at 6336 Algoa Road, Jefferson City, Missouri 65101, an employee was exposed to a caught-in hazard. A review of energy control procedures was not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources. 29 CFR 1910.147(e)(2)(i): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not ensure that all employees had been safely positioned or removed from the work area: At the workplace located at 6336 Algoa Road, Jefferson City, Missouri 65101, an employee was exposed to a caught-in hazard. The employee was not adequately removed from the spiral north side conveyor work area before the conveyor belt was re-energized. An employees hand was pulled into the energized machine when wiping away excess lubrication from the unguarded conveyor chain access panel.
