29 CFR 1910.106(f)(3)(iii):Valves used for the final control for filling tank vehicles shall be of the self-closing type and manually held open except where automatic means are provided for shutting off the flow when the vehicle is full or after filling of a preset amount. a) Rail Tank Filling Station, 2501 Margaret Street, Philadelphia, Pennsylvania - The employer did not ensure that an employee continuously monitored the loading operations when loading a rail car with Phenol, a Class 4 flammable liquid.
