On or about January 25, 2022, Various Locations on the site were performing roofing work using an aerial lift without fall protection or being anchored inside of the basket. A) The employee was wearing a harness but did not have a lanyard on the northern side of the site while using a Sunbelt Aerial lift ID # 1069919. B) The employee was not wearing fall protection on the southern side of the site while using the Sunbelt Aerial lift ID # 553063
