29 CFR 1910.22(d)(2): Hazardous conditions on walking-working surfaces were not corrected or repaired before an employee used the walking-working surface again. Location: 1 Ferry Access Road, Bridgeport, Connecticut On or about January 18, 2022, the elevator was not guarded or repaired when it was found to be not working properly. Employees were operating the elevator to access building floors; elevator was stopping at approximately 18 inches above the outer floor surface exposing employees, when stepping out of the elevator, to tripping hazards and serious injuries.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. Location: 1 Ferry Access Road, Bridgeport, Connecticut (2nd Floor) Employer did not report to OSHA within 24 hours that an employee was hospitalized due to a workplace incident when the employee sustained severe injuries to his left knee and left shoulder.
