29 CFR  1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage:  At a work site located at 301 SW Eagles Parkway, Grain Valley, MO, 64029, employees were exposed to electrical hazards.  The employer exposed employees to electrical shock and electrocution hazards by allowing the use of electrical extension cords that were in disrepair, as well as a circular saw that had an improperly repaired power cord and a missing grounding prong

29 CFR  1926.1053(b)(1): Where a portable ladder was used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a work site located at 301 SW Eagles Parkway, Grain Valley, MO, 64029, employees were exposed to potential fall hazards.  Employees were exposed to fall hazards due to the employer allowing for the improper use of a portable extension ladder.  Employees utilized a portable extension ladder to access the second level of the structure with the ladder side rails extended less than 3 feet above the upper landing surface.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems:  At a worksite located at 301 SW Eagles Parkway, Grain Valley, MO, 64029, employees were exposed to potential fall hazards. Employees engaged in residential framing activities that exposed them to fall hazards of approximately  23 feet from the roof structure to the ground below, the employer failed to ensure that the employees were using adequate fall protection.  Unguarded openings from the roof to the second level were also present in the location near where employees were working and created an estimated 9 foot fall hazard.   EBR Framing was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1330355, citation number 1, item number 1 and was affirmed as a final order on 01-17-2019, with respect to a workplace located at 8875 Maurer Ct., LENEXA, KS 66219.

29 CFR  1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  At a work site located at 301 SW Eagles Parkway, Grain Valley, MO, 64029, employees were exposed to hazards presented by poor housekeeping practices.  The employer exposed employees to slip, trip, and fall hazards by allowing them to work in an area neighboring a large pile of debris that included scrap lumber with protruding nails.
