On or about November 15, 2021 and times prior there to, the employer did not provide documentation to substantiate the existence of a fall protection training program and evidence of any training being communicated to the employees being exposed to fall hazards.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about November 15, 2021 and times prior there to, the employer did not ensure portable ladders being used by employees to access an upper landing (roof) extended at least three feet above the upper landing surface at the following instances on the residential home:  a) Ladder used to access roof that was facing the street b) Two ladders used to access the roof on the west side of the house c) Ladder used to access the roof at the rear of the house.

On or about November 15, 2021 and times prior thereto, the employer did not provide documentation to substantiate the existence of a training program for employees that used ladders as required by job duties and would enable the employees to recognize hazards and how to mitigate hazards associated with ladder usage.
