At a worksite located at 1710 Carter's Landing, Lot 54, in Jacksonville, Florida 32211: On or about January 31, 2022, the employer exposed an employee to eye injury hazards, in that eye protection was not utilized by the employee who was operating pneumatic nail guns to nail down underlayment paper, while performing framing operations.

On a 6:12 pitched roof, at a worksite located at 1710 Carter's Landing, Lot 54, in Jacksonville, Florida 32211: The employer exposed an employee to fall hazards, in that, the employee nailing underlayment paper on the roof was being exposed to falls ranging from 8 to 16-foot to the ground below, in that fall protection was not utilized, while performing framing operations.
