29 CFR 1904.40(a): The employer did not provide an authorized government representative the 1904 records within the four business hours:  On or about 02-01-2022 and times prior thereto, located at NAVFAC Mid-Atlantic NSA Hampton Roads - NMCP, Building 273, Portsmouth, VA 23702:   The OSHA 300 Logs and 300A Summary, or equivalent forms, were requested in-person during the Opening Conference on February 01st, 2022 at 10:30 AM and they were provided to the authorized government representative on February 01st, 2022 at 9:47 PM.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

On or about 09-21-2021 to 09-27-22021, and times prior thereto, located at NAVFAC Mid-Atlantic NSA Hampton Roads - Naval Medical Center Portsmouth, Portsmouth, VA 23702 on the 15th floor, HVAC Chiller rooftop area:  Employee(s) were exposed to a fall hazard in excess of 14 feet after climbing and using a fly section of an extension ladder.  The fly section had been previously removed from the base section and was being used as a single straight ladder and not as designed.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

On or about 09-21-2021 to 09-27-22021, and times prior thereto, located at NAVFAC Mid-Atlantic NSA Hampton Roads - Naval Medical Center Portsmouth, Portsmouth, VA 23702 on the 15th floor, HVAC Chiller rooftop area:  Employee(s) were exposed to a fall hazard in excess of 14 feet after climbing and using a fly section of an extension ladder that had rubber tread riveted on the "foot" area, whereas the rubber tread had separated and was not secured to the ladder.  This rubber tread addition was not original manufactured design.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

The parapet wall was estimated to be less than 36 inches in height.  Abatement certification required within 10 days after abatement date. On or about 09-21-2021 to 09-27-22021, and times prior thereto, located at NAVFAC Mid-Atlantic NSA Hampton Roads - Naval Medical Center Portsmouth, Portsmouth, VA 23702 on the 15th floor, HVAC Chiller rooftop area:  Employee(s) were exposed to a fall hazard in excess of 14 feet while changing aircraft warning fixture lightbulbs without an adequate form of fall protection.  The parapet wall was estimated to be less than 36 inches in height.  The employee used a lightening grounding cable that was bolted to the top concrete cap of the parapet wall as a fall protection anchor point.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

Employee used a lightening grounding cable that was bolted to the top concrete cap of the parapet wall as an anchor point that was not a certified anchor point to support at least 5000 pounds for each employee attached.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
