29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: (a) In building 518 at Hart Tie & Lumber Company; the infeed lift of the Quick Rip multi-rip saw was being utilized with an electrical junction box that was missing a cover plate, creating an exposure to electrical shock hazards and a risk of fire due to a build-up of dust in the box.
