Murray & Stafford, Inc., at 14802 N. Grant Street, Thornton, CO 80023: On or before February 1, 2022, Murray & Stafford Inc., as the exposing and controlling employer, did not protect its employees and four employees from Rogers and Sons Inc., subcontractor from slips and fall hazards. The employer did not inspect and mitigate the ice on the walkway to the rooftop work area.  One worker slipped on ice and was injured. This condition exposed employees to broken bones and head injuries.
