29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Facility- 155 South Rebecca Street, Saxonburg, PA: On or about February 8, 2022, employees inserted an 1/8" piece of phenolic sheet to wedge the switch blades and connections tight inside of an electrical transfer box. The phenolic sheet was not listed and labeled for this type of electrical work.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: (a) Facility- 155 South Rebecca Street, Saxonburg, PA: On or about February 8, 2022, a broke down powered industrial truck was being stored in front of an electrical disconnect, not allowing employees with sufficient access to the disconnect.
