29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Jobsite, Press Room, 155 South Rebecca Street, Saxonburg, PA 16056: On or about October 24, 2022, an employee received amputation injuries while operating a Stokes brand powder compacting press (20 ton capacity) designated as '73'. The press was not adequately guarded in that a gap measuring approximately 6 inches in width and 6.5 inches in height existed between the guard and the side of the machine near the output chute.
