29 CFR  1910.215(a)(1):   Abrasive wheel(s) used on grinding machinery were not used with safety guard(s):  a)  Room 112:  On or about January 9, 2022, a Wen WEN 4276 2.1 Amp Motor -6 Inch Bench Grinder 3450  RMP 6"x3/4 Inch wheels, 1/2 arbor was missing a guard exposing employees to caught-in and struck by hazards.

29 CFR  1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire:   a)  Hanger adjacent to electrical panel #36:  On or about January 9, 2022, Several M-Cylinder Oxygen tanks were stored improperly and in close proximity to an electrical panel, cleaning supplies and mounted to a screen fence with a truckers tiedown, exposing employees to explosions, fire, and burn hazards.

29 CFR  1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a)  Room 112:  On or about January 9, 2022, a relocatable power tap (surge protector) was permanently mounted to the wall of a filing cabinet subjecting employees to fire and electrical shock hazards.  b)   Building 47430 Admin Area:  On or about January 9, 2022, a Relocatable Power Tap was used in conjunction with another extension cable creating an electrical hazard, exposing employees to electrical shock and fire.

29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater. In all cases, the working space shall permit at least a 90-degree opening of equipment doors or hinged panels.  a) Hanger Panel #36: On or about February 9, 2022, several oxygen cylinders were stored in front of an electrical panel restricting the working space that did not allow the panel be readily accessible in the event of an critical emergency, exposing employees to electrical hazards.  b) Near or Room 100: On or about February 9, 2022, a maintenance lift, broom, and a box of ice melt  was stored in front of an electrical panel restricting the working space and did not allow the panel be readily accessible in the event of an electrical emergency, exposing employees to electrical hazards.
