The SALA SecureaSpan Horizonal Lifeline System was not installed as the manufacturer's instructions and used under the supervision of a qualified person.

The retractable lifelines and the horizontal lifeline system did not limit free fall distance to 6-feet. b) Worksite, 211 North Main Street, Stillwater, New York, Roof of Facility: On or about February 09, 2022, an employee was using a personal fall arrest system from the roof of a commercial building approximately 36-feet above the ground. The employee was tied off approximately 50-feet from the anchor point and approximately 10-feet from the leading edge exposing the employee to a free fall of more than 6-feet. c) Worksite, 211 North Main Street, Stillwater, New York, Roof of Facility: On or about February 09, 2022, an employee was using a personal fall arrest system from the roof of a commercial building approximately 36-feet above the ground. The employee was tied off approximately 65-feet from the anchor point and approximately 10-feet from the leading edge exposing the employee to a free fall of more than 6-feet.
