29 CFR 1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): At the workplace located at 989 East Commercial Street, Mansfield, Missouri 65704, employees were exposed to struck-by powered industrial truck hazards. The Clark electric forklift, serial number ECG358-0297-6754FB that was in operation, had the following items that were found to be missing, or defective and in need of repairs: 1. Two missing instrument or control knockouts on the dashboard. 2. Two missing directional indicators on the forklift lever controls. 3. Two directional indicators on the forklift lever controls were illegible. 4. Damaged negative electrical cable on battery charge wiring.
