29 CFR 1926.95(a):Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: (a) On or about February 8, 2022, the employer did not ensure that protective equipment for eyes, face, and extremities was provided to workers involved in live-electrical work activities. An employee of Bear Electric, Inc. was exposed to shock and arc-flash hazards while landing new service feeders on an energized 480-Volt meter stack. The worker suffered burns to his hand, neck and face as a result of an arc-flash caused by contact with live components. The employee was not provided with voltage-rated gloves, arc flash clothing, or a face shield to protect him from the hazards. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) On or about February 8, 2022, Bear Electric, Inc. permitted employees to work in proximity to electric power circuits and did not protect the employees against electric shock by de-energizing or effectively guarding the circuits by insulation or other means. An employee of Bear Electric, Inc. was exposed to shock and arc-flash hazards while landing new service feeders on an energized 480-Volt meter stack. The worker suffered burns to his hand, neck and face as a result of an arc-flash caused by contact with live electrical components. The employer did not de-energize the equipment and did not implement lockout/tagout procedures before allowing employees to work on electrical equipment. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
