29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. a) On or about February 10, 2022, and at times prior thereto the controlling employer, CSDI Construction did not ensure that its sub-contractor(s) protected their employees. Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact from falling and/or flying objects.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On or about February 10, 2022 and at times prior thereto the controlling employer, CSDI Construction did not ensure that its sub-contractor(s) protected their employees from a fall hazards of approximately 33 feet while accessing the roof to apply siding. b) On or about February 10, 2022 and at times prior thereto; the controlling employer, CSDI Construction, Inc., did not ensure that its sub-contractor(s) protected their employees from fall hazards of approximately 33 feet while accessing the roof to perform work on the HVAC system.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail: a) On or about February 10, 2022 and at times prior thereto temporary handrails were not in place on the permanent stairways throughout the building. CSDI Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1052(c)(1)(i), which was contained in OSHA inspection number 1482500, citation number 1, item number 2 and was affirmed as a final order on September 23, 2020, with respect to a workplace located at 808 Cleveland Blvd, Caldwell, Idaho 83605.
