1910.178(a)(1): This section contains safety requirements relating to fire protection, design, maintenance, and use of fork trucks, tractors, platform lift trucks, motorized hand trucks, and other specialized industrial trucks powered by electric motors or internal combustion engines. This section does not apply to compressed air or nonflammable compressed gas-operated industrial trucks, nor to farm vehicles, nor to vehicles intended primarily for earth moving or over-the-road hauling. On or about January 12, 2022, at an establishment located at 2099 Badgerland Drive in Green Bay, Wisconsin, in the dumping bays; the employer did not ensure employees that were operating snorkels and working in the area were protected from being struck and crushed by tipping snorkels and semitrailers. While dumping the semitrailer with the 5th Wheel Hoist, stability of the system was affected by load conditions such as, but not limited to, frozen contents and/or being overloaded. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. On or about February 15, 2022, at an establishment located at 2099 Badgerland Drive in Green Bay, Wisconsin, on the second floor near the static screen towers and the Panel Operator's Station; employees walking through the area to adjust valves or scrape the static screen towers were exposed to slip hazards from water leaking in through a hole in the ceiling and pooling on the floor.

29 CFR 1910.305(j)(1)(iv): Fixtures installed in wet or damp locations shall be identified for the purpose and shall be so constructed or installed that water cannot enter or accumulate in wireways, lampholders, or other electrical parts. On or about February 15, 2022, at an establishment located at 2099 Badgerland Drive in Green Bay, Wisconsin, on the second floor near the static screen towers and the Panel Operator's Station; a string of portable construction lights and a static screen motor installed in a wet and damp location were not identified for the purpose and were not constructed or installed so that water could not enter or accumulate in wireways, lampholders, or other electrical parts. Panel operators passing through the area to adjust valves or scrape the static screen towers were exposed to electrical shock hazards from water leaking in through a hole in the ceiling and onto a string of portable construction lights and a static screen motor.
