29 CFR 1910.178(q)(1):Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: Employees are exposed to fire and explosion hazards in that the compressed gas cylinder was missing a safety strap. On or about February 16, 2022, employees operated a forklift with a bracket that holds the compressed gas cylinder onto the forklift was broken allowing for the tank to move while in operation.

29 CFR 1910.212(a)(2):Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: Employees are exposed to caught-in-between and amputation hazards on the Minster Power Press #1 in that a guard that is required on the machine was missing. On or about February 16, 2022, Employees were operating the Minister Power Press where a guard was missing from the machine, leaving moving chains and parts exposed.
