29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: 15 Mallowhill Rd., Springfield, MA: On or about February 17, 2022, an employee performing roofing work was observed climbing an aluminum extension ladder that the employer did not ensure was positioned so that the ladder extended 3 feet past the upper landing surface (roof). Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19 (d) (1).

29 CFR 1926.1053(b)(5)(i):Non-self-supporting ladders were used at an angle such that the horizontal distance from the top support to the foot of the ladder was greater than one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support): 15 Mallowhill Rd., Springfield, MA: On or about February 17, 2022, the employer had not ensured that an employee using a portable extension ladder to access an upper level was installed in manner in which the working angle did not exceed a ratio of 4:1 or one quarter the working length of the ladder. The ladder was set up at a 2.6:4 angle exposing employees to ladder displacement and fall from height hazards. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19 (d) (1).

Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19 (d)(1).
