29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  On or about February 17, 2022, and at times prior thereto, an employee was working on a multi-level roof directly below other employees who were installing roof tiles at heights greater than 14 feet, exposing employee to struck-by hazards.

29 CFR  1926.451(a)(6):Scaffolds were not constructed in accordance with the design by a qualified person.  On or about February 17, 2022, and at times prior thereto, employees were utilizing and riding inside a site-made wooden trash box at heights up to approximately 14 feet on a Rough Terrain Truck, Model G6-42A, that was not designed by a qualified person, exposing workers to a fall hazard.

29 CFR  1926.451(c)(2)(v):Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied.  On or about February 17, 2022, and at times prior thereto, employees were utilizing a site-made wooden trash box that was not secured to the forks of a Rough Terrain Truck, Model G6-42A, exposing employees to fall hazards.

On or about February 17, 2022, and at times prior thereto, employees were operating a Telehandler Rough Terrain Truck, Model G6-42A without training or certifications, exposing workers to fall and struck-by hazards.
