29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about February 14, 2022, the employer did not ensure employees were in use of eye protection when operating a pneumatic nail gun.  This exposed the employee to struck-by hazards.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about February 14 , 2021, the employer did not ensure employees performing roofing work, from the roof of a residential building, were in use of fall protection.  This exposed the employees to a fall hazard of more than six (6) feet to the ground.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     On or about February 14, 2021, the employer did not ensure the ladder side rails extended at least three (3) feet above the landing.  This exposed the employees to a fall hazard.
