29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  a-76 E Mahwah Road, Mahwah NJ 07430 (Residential Roof): CSHO observed an employee accessing a roof using a portable extension ladder exposed to fall hazards of approximately 10ft to the asphalt below as the ladder side rails were not raised 3ft above the upper landing surface. Condition noted on or about 02/16/2022.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)(1)  a-76 E Mahwah Road, Mahwah NJ 07430 (Residential Roof): CSHO observed an employee engaged in the removal of pump jack supports  positioned at the unprotected roofs edge in which a means of fall protection had not been utilized exposing the employee to fall hazards of approximately 14ft to the asphalt below. Condition noted on or about 02/16/2022.
