On or about February 18, 2022, with respect to an establishment located at 1101 Perry St, Algoma, WI 54201, employees were exposed to fall hazards of 5 feet and 7.5 feet due to several open areas in the guardrail system around the lower pit of the large Horizontal Boring Machine in the Saw Area.

29 CFR  1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.   On or about February 18, 2022, with respect to an establishment located at 1101 Perry St, Algoma, WI 54201, the employer had not developed written procedures for service and maintenance activities on machinery such as presses, shears, and CNC machines, in the production facility .   Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or re-accumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance.

A thorough inspection of all ropes shall be made at least once a month and a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier for the ropes which were inspected shall be kept on file where readily available to appointed personnel. Any deterioration, resulting in appreciable loss of original strength, shall be carefully observed and determination made as to whether further use of the rope would constitute a safety hazard. Some of the conditions that could result in an appreciable loss of strength are identified in 1910.179(m)(1)(i)-1910.179(m)(1)(vi). 29 CFR  1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  On or about March 1, 2022, with respect to an establishment located at 1101 Perry St, Algoma, WI 54201, employees were exposed to hazards associated with moving machine parts when: a) the key controlling the door interlock for the Daewoo Puma 18 CNC Lathe, Serial #PM180010 was left in the machine and allowed an employee to turn off the interlock function. b) employees were using the Bayleigh Type HSP-176M Hydraulic Shop Press, Serial #12 P0033 in the Welding Bay. The Press was operated by a single lever that allowed access to the point of operation while the ram was in motion.  c) an opening in the on the frontside of the Scotchman Ironworker, Model D070-24M, Serial #1280M0605 in the Fabrication area allowed employees to access the point of operation while the machine was in motion.   d) employees using the AccurPress 130 Ton Hydraulic CNC Press Brake, Model 713010, Serial #10669  were not protected from the moving dies of the presses by any guarding. An employee was observed with their hands on the dies as the machine performed a bend.  e) employees using the AccurPress 175 Ton Hydraulic Press Brake, Model 7017512, Serial #8486  were not protected from the moving dies of the presses by any guarding. f) employees using the AccurShear Shear, Model 625010, Serial #5033 were able to access the hold downs of the shear due to the fixed barrier guards having openings that were wide enough to allow access to the points of operation.

Joining lengths of work and electrode cables shall be done by the use of connecting means specifically intended for the purpose. The connecting means shall have insulation adequate for the service conditions.  [55 FR 13709, Apr. 11, 1990; 61 FR 9227, March 7, 1996; 70 FR 53929, Sept. 13, 2005]  On or about March 1, 2022, with respect to an establishment located at 1101 Perry St, Algoma, WI 54201, employees were exposed to potential shock and electrocution hazards when the negative weld cable for a Miller D-74S  Wire Feeder was spliced and missing insulation that exposed employees to bare conductors.
