29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300. On or about February 27, 2022, through July 26, 2022, at an establishment located at 1101 Perry Street in Algoma, Wisconsin; the employer did not update and/or provide the Section (F) required injury description for a work-related fatality. On February 15, 2022, an employee was hospitalized following a work-related incident on the same day. On or about February 18, 2022, the employee passed away from the injuries sustained from the work-related incident. The employer was updated on the deceased status of the employee on February 20, 2022.

29 CFR 1904.41(a)(2):Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form. On or about July 26, 2022, at an establishment located at 1101 Perry Street in Algoma, Wisconsin; the employer failed to electronically submit information from the OSHA Form 300A Summary of Work-Related Injuries and Illnesses for the preceding calendar year by March 2, 2022.
