At the facility located at 1695 Mills Street, Green Bay, WI, 54302, the employer did not ensure that each powered industrial truck operator that was involved in an accident or near-miss incident was retrained and evaluated on the effectiveness of that training, exposing employees to struck-by hazards in the following instances:  a) The employer did not ensure that one powered industrial truck operator was provided refresher training and an evaluation of the effectiveness of that training following an accident on or about January 4, 2022. b) The employer did not ensure that one powered industrial truck operator was provided refresher training and an evaluation of the effectiveness of that training following a near miss on or about January 17, 2022. c) The employer did not ensure that one powered industrial truck operator was provided refresher training and an evaluation of the effectiveness of that training following an accident on or about January 24, 2022. d) The employer did not ensure that one powered industrial truck operator was provided refresher training and an evaluation of the effectiveness of that training following an accident on or about February 7, 2022.
